I understand that non-identifiable information I provide may be used to help publicly track where the outbreak may be spreading. Further, I understand that this form is not a legal/medical/government entity but a community initiative to help share information with our State Government. I consent to the sharing of non-identifiable/anonymous data to aid researchers studying COVID-19. I recognize that if sick, I must contact my local medical provider and professional immediately.
